question mark and checkmark against a yellow background with the words Community Health Survey

The Community Health Equity Survey is an opportunity for residents to share their experiences and priorities to help shape the future of their health. The Massachusetts Department of Public Health will use survey results to improve its programs, make decisions about funding and resources, and support policies to improve health inequities. They will also share the survey results with partners across Massachusetts to support their vital work to improve health in their communities.

The survey will be open through early Fall 2023. Anyone aged 14 and older can take the survey. It takes approximately 15 to 25 minutes to complete and is available in 11 languages.

Taking the survey is voluntary and easy! All responses are anonymous. Just click on this link to get started
